Enter Last Name.  To further refine your search enter the last name and the first three characters of the first name followed by an *.  For example, if you are searching for Harry Smith, you would enter 'Smith' in the Last Name field and 'Har*' in the First Name field.

After entering the name, choose from the resulting list by selecting (clicking) on the name you want verified.  You may add multiple licensees.
